be discussed at your initial visit. |

appreciate payment for services at
the time they are rendered. If this
is not possible, special financial
arrangements can be discussed with
Barbara, our office manager. |

If you
have dental insurance, please
bring your completed form to your
first visit. We will gladly process your
forms for you. Since benefits vary from
plan to plan, your dental insurance
booklet (from the insurance company)
should provide you with information as
to what services are covered. |
